I recently moved to Spain and caught a cold with a headache and runny nose. When I went to buy some cold medicine, I discovered that pharmacies here are completely different from what I was used to. After doing some research, I’m sharing what I’ve learned about buying medicine and medical supplies in Spain to help anyone else in need.
Pharmacies in Spain: Farmacia vs. Parafarmacia
First, it’s crucial to understand the difference between these two types of stores!
Farmacia: This is the traditional pharmacy, easily identified by the green cross sign outside. It’s the only place legally authorized to sell both prescription drugs and over-the-counter (OTC) drugs. Information on current [Spanish medical supplies] can be found online. A professional pharmacist must always be on duty to provide expert advice on medication. If you have a doctor’s prescription, this is where you need to go.
Parafarmacia: These stores are often located inside large supermarkets or as standalone shops. They are only permitted to sell items like dietary supplements, mother and baby products, skincare, and hygiene items. In short, they sell health-related products that are not classified as “medicines.” This is the place for vitamins, sunscreen, or baby formula, but you won’t find painkillers or antibiotics here.

How to Buy Common Over-the-Counter (OTC) Drugs?
In my case, it was just a common cold, so no doctor’s visit was necessary. You can go directly to a Farmacia and describe your symptoms to the pharmacist, for example, “dolor de cabeza” (headache), “fiebre” (fever), or “tos” (cough). They will recommend a suitable OTC medicine based on your condition. Many pharmacists in Spain are very patient, and some can speak English.
Here are a few common OTC drugs I learned about, partly through researching [Spain’s healthcare spending]:

Note: Even with OTC drugs, always follow the pharmacist’s advice and the product instructions. Do not self-medicate without proper guidance!
About Prescription Drugs and Online Pharmacies
If you need prescription medication, you must first see a doctor to get a prescription slip (receta médica). You can then take it to any Farmacia to purchase the medicine. Just as finding the right Spanish medical attire is important for professionals, getting the right prescription is key for patients. If your prescription is from the public healthcare system, your co-payment for the medicine will be very low, making it highly affordable.
Many online pharmacies now operate in Spain, offering a convenient way to buy Parafarmacia products and some OTC drugs. However, please be aware that under Spanish law, it is illegal to sell prescription drugs online! If you find a website claiming to sell prescription medicine without requiring a prescription, it is 100% a scam. Please be extremely cautious.
Spain’s pharmaceutical system is very well-regulated. For minor ailments, head to a Farmacia and speak to a pharmacist. for more serious health issues, visit a doctor to get a prescription. Hopefully, you’ll stay healthy in Spain and won’t need this information at all!
