Having just arrived in Spain, I’d always heard about its free public healthcare, but I was never quite sure how to access this benefit. I finally decided to apply for my health card recently and found the process much simpler than I expected! Here, I’ll share my personal experience with Spanish public healthcare to help anyone else who needs it.

Who Can Apply for the Free Health Card?
Many people think that only those who are working or have a residency permit can apply, but that’s not entirely true. Under current regulations, as long as you are legally registered at an address in Spain (empadronado) and do not have healthcare coverage from another country, you are theoretically entitled to apply. I applied with my student residency permit, and the staff didn’t give me any trouble.
The most crucial point is this: You must first go to the Social Security office (Tesorería General de la Seguridad Social - TGSS) to obtain a document called ‘Certificate of the Right to Healthcare’. Without this certificate, you will be turned away if you go directly to the health center.
The entire process can be broken down into two steps: getting the certificate from the Social Security office, and then applying for the card at your local health center (centro de salud). It sounds simple, but make sure you have all the necessary documents to avoid a wasted trip.

Important Note: It’s best to book an appointment (Cita Previa) online before going to the Social Security office. If you just show up, it’s very likely they won’t be able to assist you with healthcare in Spain. Applying at the health center is more straightforward; you can usually do it at the reception desk. I waited about 20 minutes, and the staff issued me a temporary paper certificate. The official card will be mailed to your home in a few weeks. Now, seeing a family doctor or going to a public hospital is much more convenient!
