Shortly after arriving in Spain, I was keen to get my health card sorted. After all, when you’re abroad, having coverage for even a minor headache or cold provides great peace of mind. After doing some research and asking a few people who’ve been here for a while, I finally got my card, and it was completely free! I’m sharing my experience with the Spanish public healthcare system here to help anyone who might need it.
Who is eligible to apply?
First of all, not everyone can apply directly. The most crucial requirement is that you must have legal residency in Spain and be contributing to social security, which is linked to free healthcare in Spain.
Simply put, you should be eligible if you meet one of the following conditions:
- Individuals who are currently employed
- Individuals receiving unemployment benefits
- Individuals on a dependent visa, registered as a beneficiary under a family member who has social security coverage for healthcare in Spain
- Holders of certain types of student visas
Don’t assume that having a residency card automatically grants you healthcare access. Contributing to social security is the key
!
Application Steps
The entire process can be broken down into two main steps: obtaining the ‘right to healthcare’ document from the Social Security office, and then applying for the health card at your local health center.
Step 1: Obtain the ‘Right to Healthcare’ Document from the Social Security Office
This step is to prove your entitlement to public healthcare. You need to go to the Social Security office (Tesorería General de la Seguridad Social - TGSS)
corresponding to your home address. It’s highly recommended to book an appointment (cita previa) online beforehand; otherwise, you could be waiting forever.
Here are the documents you’ll need to prepare:
| Document Name | Spanish Reference | Notes |
| Application Form | Modelo TA.1 | You can download it from the Social Security website or fill one out on-site. |
| Original and photocopy of your residency card | TIE / Certificado de Registro de Ciudadano de la Unión | Must be valid. |
| Proof of address (Empadronamiento) | Certificado de Empadronamiento | Usually required to be issued within the last 3 months. |
| Original and photocopy of your passport | Pasaporte | Photocopy the entire passport, just in case. |
| Work contract or social security life report | Contrato de Trabajo / Vida Laboral | To prove you are contributing to social security. |
Once you submit the documents, the staff will review them. If everything is in order, they will print a document for you on the spot called the “Documento acreditativo del derecho a la asistencia sanitaria”. This is what is commonly known as the ‘right to healthcare’ document. Once you have this paper, you’re 80% of the way there!

Step 2: Go to Your Local Health Center (Centro de Salud)
Take the document from Step 1, along with your residency card and proof of address (empadronamiento), to your designated local health center (Centro de Salud). You can find the correct one by searching “Centro de Salud” + your postal code on Google Maps. At the reception, hand over your documents. The staff will enter your details into the system and inform you whether the health card will be mailed to your home or if you should come back to pick it up later. In my case, I received it in the mail after about two weeks.
Once you receive the card, it will have your personal details and the information of your assigned GP (General Practitioner). From then on, whenever you need to see a doctor, you just need to make an appointment with your GP at this health center. Although the process requires visiting two different places, it’s quite straightforward as long as you have all your documents ready. I hope my experience is helpful to you!
