Having recently arrived in Spain, I kept hearing how important the health card is as the key to accessing public healthcare. After putting it off for a while, I finally decided to get it sorted. I found that many regions now allow you to apply online, which is incredibly convenient and saves the hassle of queuing at an office. The entire process is not complicated. The main task is to first get a Social Security number (Número de Afiliación) and then apply for the health card on the health system website of your autonomous community. If you’re employed, your company usually handles this. If you’re a student or a dependent, you might need to apply for one yourself on the Social Security website. This is a crucial prerequisite! 1. Prepare Your Documents
Before you start, prepare digital copies of these documents, usually in PDF or JPG format:
- A scan of the front and back of your NIE/TIE card
- Proof of address (Certificado de Empadronamiento), which must be recent, preferably issued within the last three months.
- The document from the Social Security office confirming your right to healthcare (Documento acreditativo del derecho a la asistencia sanitaria), which you can download from their website.
- The application form, which can usually be downloaded from the health system’s website.
2. Find the Application Portal for Your Autonomous Community

Once you’ve found the correct website, look for an option like “Solicitud de Tarjeta Sanitaria” or something similar. Click on it and follow the step-by-step instructions to fill in your personal information and upload your documents. The process is guided, so it’s hard to go wrong. After submitting, you’ll receive a confirmation receipt. Be sure to save it, as you can use it to track your application status. The physical card will be mailed to your address in a few weeks. While waiting, if you need urgent medical care, you can go to a local health center (centro de salud) with your application receipt and ID, and they will assist you.
