My family has recently been discussing a potential move to Italy, as my partner has a great job opportunity there. We’ve been settled in Barcelona for several years now, so we’re quite familiar with the Spanish healthcare system. However, we have no idea what it’s like in Italy. So, I wanted to start this thread to discuss which is actually better: Spanish healthcare or Italian healthcare? I’ll also share some of my personal experiences to hopefully get the ball rolling.
My Personal Medical Experiences in Spain
When I first arrived in Spain, after getting my municipal registration (Padrón), I went to the local Centro de Salud to register with a family doctor (GP). I feel that the Spanish system is very clear in this regard; everyone is assigned a specific GP who you see first for minor ailments. If they can’t handle it, they refer you to a specialist. I’ve been a few times for allergy issues, and the appointment wait time was about a week, which is quite acceptable. The GPs are very responsible and ask detailed questions. Later, I needed an X-ray for a sports injury and was referred to a major hospital. That wait was much longer—it took almost a month to get an appointment. My overall impression is that it’s very convenient for routine minor illnesses, but the efficiency for specialist examinations is definitely a drawback.
Public vs. Private Healthcare
Speaking of efficiency, we have to talk about the difference between public healthcare and private insurance. Many of my friends, including ourselves, use both. Public healthcare offers broad coverage and is mostly free, but the key word is ‘waiting’. Private insurance is about paying to save time; booking specialists and tests is usually much faster. For example, when my wife needed to see a gynecologist, she used our private insurance and got an appointment with a specialist in just a few days. Through the public system, it probably would have been a few months. I’ve heard that Italy has a similar model, but I’m not sure about the prevalence and cost of private insurance there. Are there any friends here who know about the situation in Italy?

I’m personally quite satisfied with Spain’s healthcare system, especially the family doctor model, which provides a great sense of security. Although the public system’s waiting times are a major issue, having private insurance as a supplement generally meets most needs, which is an important factor in any healthcare comparison. My biggest concern now is the situation in Italy. If the healthcare experience would be a downgrade, we might have to reconsider the move.
