Having just arrived in Spain, I wasn’t very familiar with the healthcare system here. Recently, I had a sudden medical emergency and, due to the urgency, went to the nearest private hospital, which cost me a fair bit of money. I was worried that the money was gone for good. However, after some research and going through the process, I managed to get most of it reimbursed! Today, I’m sharing the specific process for ‘Reintegro de Gastos Médicos’ (Medical Expense Reimbursement) to help anyone who might need it.
What is Medical Expense Reimbursement?
Simply put, it’s when you go to a private hospital in certain specific situations because you’re unable to use the public healthcare service, and you can later apply to the social security system to get reimbursed for these costs. But here’s the key point: not all situations are eligible for reimbursement, and it’s not always a 100% refund, which is another aspect of the healthcare system to learn about, just like buying medicine in Spain. The Social Security office will reimburse you based on the cost of the same service within the public healthcare system; you’ll have to cover any amount exceeding that.
What Situations are Eligible for Application?
The most common situation is ‘Urgencia vital,’ which means a life-threatening emergency. For example, if you have a sudden case of acute appendicitis in the middle of the night or are in a serious accident, and there are no public hospital emergency rooms nearby, you can go to a private hospital for life-saving treatment and apply for reimbursement later. In my case, I went to a private emergency room because I had a persistent high fever, all the local clinics were closed, and I couldn’t get an appointment with my family doctor. Before applying, you must ensure your situation meets the ‘urgent and necessary’ principle.
Preparing the Application Materials
Preparing the documents is the most critical step of the entire process. If your materials are incomplete, your application will definitely be rejected. Based on my experience, you will primarily need the following:

| Justificante de pago | Proof of payment, such as a bank card transaction record. |
| DNI/NIE y Tarjeta Sanitaria | Photocopies of your ID (DNI/NIE) and your health card. |
Once you have all these materials ready, you can feel more confident. I had to make an extra trip to the hospital just to get the ‘informe médico’, so remember to ask for it on the spot after your consultation. Also, it’s best to make a copy of all documents for your own records.

Submission and Waiting
Once your documents are ready, you can submit them to your local Social Security office (Instituto Nacional de la Seguridad Social - INSS). Find a nearby office, book an appointment (‘Cita Previa’), and go there with all the original documents and their photocopies. The staff will check your documents, take the originals, and give you a receipt. Then comes the long wait… Eventually, the reimbursed money will be directly deposited into the bank account registered with your social security.
