When you first arrive in Spain, besides getting your residency permit and bank account, one of the most important things to sort out is your health card! With it, visits to public hospitals are essentially free, which can save you a lot of money and give you great peace of mind.
I just went through the process last month, so I wanted to share a complete walkthrough while it’s still fresh in my mind, hoping to help others who need it. The whole procedure isn’t complicated as long as you have all the necessary documents and follow the steps.
Step 1: Get Your Social Security Number
This is a prerequisite for applying for the health card and just as important as knowing about buying medicine in Spain. If you are a student with a qualifying health insurance plan, your insurance company might handle this for you, similar to how Spanish medical reimbursement works. However, most people, especially those who are working, will need to apply for it themselves at the Social Security office (Tesorería General de la Seguridad Social).
The required documents are usually:
- Original and photocopy of your passport and residency card (NIE)
- Completed application form Modelo TA.1
- Proof of address (empadronamiento), preferably issued within the last 3 months
These days, many offices require you to book an appointment online first; just showing up might be a wasted trip. It’s best to be prepared for all medical system interactions, including understanding how to get a [medical refund].
Step 2: Get Recognition of Your Right to Healthcare
After getting your social security number, the next step is to go to the National Social Security Institute (INSS) to certify your right to public healthcare. For this, you will also need to book an appointment (Cita) online in advance.
When you go to the INSS, bring your social security number, passport, NIE, and proof of address (empadronamiento). Once the staff reviews and approves your documents, they will give you a confirmation document stating that you are entitled to public healthcare. This paper is very important, so make sure to keep it safe!
Step 3: Go to Your Local Health Center to Apply for the Health Card

The final step! Take the confirmation document from the INSS, along with your passport, NIE, and proof of address, to the health center (Centro de Salud) assigned to your residential address. You can find the one closest to you by searching for “Centro de Salud” on Google Maps.
Once at the health center, tell the receptionist you want to apply for the “Tarjeta Sanitaria”. They will ask you to fill out a form and will verify your information. After you’re done, some centers will give you a temporary paper version of the health card on the spot, which includes your personal details and assigned family doctor. The official plastic card will be mailed to your home address in a few weeks.
