When we first arrived in Spain, we often heard that the healthcare benefits, like the Spanish health card, were excellent and world-class. Recently, we’ve seen news and reports again mentioning Spain’s global healthcare ranking, and the opinions seem quite varied—some say it’s in the top ten, while others claim it has fallen out of the top twenty. So, we decided to do some research and, drawing from our own medical experiences, discuss with everyone: what is the actual standard of the healthcare services we enjoy in Spain?
What Do the Authoritative Rankings Say?
When it comes to rankings, the most famous one is from the World Health Organization, but the data for things like Spanish health insurance is a bit outdated. In recent years, more relevant references include Bloomberg’s ‘Global Health Efficiency Index’ and CEOWORLD magazine’s ‘Health Care Index.’ In these lists, Spain frequently ranks among the top, especially for its healthcare efficiency, consistently appearing in the global Top 10. This means it achieves significant results with relatively low expenditure, and the nation’s life expectancy is also very high. This is largely thanks to its universal public healthcare system.

Public vs. Private: A Different Experience
However, rankings are macroeconomic, while the experiences of ordinary people are what truly matter. In Spain, healthcare is divided into public and private systems. As long as you have legal residency and social security contributions, you can access free public healthcare. Its advantages include broad coverage, strong protection for major illnesses, and low medication costs. But its drawback, as many know, is that it’s slow… slow… slow… Booking an appointment with a family doctor for Spanish healthcare might take a week or two, and waiting for specialists and tests can take several months. Emergency rooms are also often overcrowded. Therefore, many people choose to purchase private health insurance for the sake of speed, allowing them to see specialists and get tests without waiting, and the service experience is much better.
