I just moved to Spain recently and have been looking into the healthcare system here. It’s quite interesting, and I wanted to share what I’ve found and also hear your thoughts and additions.
When I first got my residence card, the first thing I did was register at my local health center for a public health card to access the public system. However, many of my friends have private insurance, claiming it’s faster and offers better service. This got me curious: what are the main healthcare brands in Spain, and what are the differences between them?
The Public Healthcare System
This isn’t really a ‘brand’ but rather a national welfare system provided by the state. As long as you have legal residency and are registered with social security, you can access it for free. Its main advantage is its comprehensive coverage—major illnesses, surgeries, and the like are almost entirely covered by the state, so you pay very little out of pocket. The downside, as many know, is that it’s slow… It’s common to wait months for an appointment with a specialist. However, for Spanish healthcare routine check-ups and emergencies, it provides solid coverage.

Major Private Health Insurance Brands
If you want to avoid long queues, need access to specific Spanish medical equipment, or prefer more personalized service, then getting private insurance is a popular choice. From my research, these are the most common ones on the market:
Sanitas: Arguably one of the most well-known private healthcare brands in Spain, it’s part of the British BUPA group. They have an extensive network, including many of their own hospitals and medical centers. Their reputation is generally excellent, and they offer comprehensive services, though their premiums are also relatively higher.
Adeslas: This company is the market leader in terms of share, backed by the powerful CaixaBank. Its main characteristic is its great value for money. It boasts a massive network of affiliated clinics and doctors, covering virtually every city in Spain. Many companies provide their employees with group insurance from Adeslas.
Asisa: This is another long-standing Spanish insurance company, founded by a cooperative of doctors. Like Sanitas, Asisa has many of its own hospitals and clinics, such as the well-known Hospital El Moncloa in Madrid. Their services are very professional, with particular strengths in certain medical specialities.
DKV: This is a German-based insurance company that is also very popular in Spain. They place a strong emphasis on preventive medicine and health management. Besides providing comprehensive [Spanish health insurance], they also offer guidance on healthy lifestyles and app-based services. They feel quite modern and are popular among younger people.
