When I first moved to Spain, I often heard that the healthcare here is free but inefficient, with emergency rooms so slow you could ‘die waiting.’ There’s some truth to that, but after living here for a while and having more interactions with the medical system, I’ve discovered that Spain is a true ‘heavyweight’ in certain high-end medical fields. Today, I want to talk about what I consider the three most impressive medical specialties in Spain and see if you all agree.
Organ Transplantation
This is probably the one you’ve heard about most. It’s no exaggeration to say Spain is number one in the world. For over thirty consecutive years, it has had the highest per-capita organ donation and transplantation rates globally. Behind this is an incredibly mature and efficient national transplant system. The entire process, from identifying potential donors, communicating with families, and procuring and allocating organs to performing the surgery, is astonishingly fast.
A relative of a friend of mine received a liver transplant here, and the time from getting on the waiting list to the surgery was much shorter than he expected. He described the feeling as a true second chance at life. So, even though we may not need it ourselves, knowing we live in a country with such a well-established system provides a great deal of peace of mind.

Cardiac Surgery
Spain’s cardiac surgery, particularly in minimally invasive procedures and the treatment of complex heart diseases, is top-tier in Europe and even globally. Many procedures that sound daunting, like aortic valve replacement or coronary artery bypass, are now often performed through very small incisions, allowing for much faster patient recovery.
I once visited the cardiology department at Gregorio Marañón Hospital for a family member’s heart palpitations, and the doctors there came across as incredibly professional and confident. They take a lot of time to explain the condition and the pros and cons of various treatment options. I later learned that several Spanish hospitals, such as Hospital Clínic in Barcelona and La Paz Hospital in Madrid, are at the forefront of cardiac surgery. They excel at integrating research with clinical practice, meaning new technologies are quickly made available to patients.
Oncology
Cancer is a topic that scares everyone. However, Spain is doing remarkably well in oncology, especially in cutting-edge fields like targeted therapy and immunotherapy. The Spanish National Cancer Research Centre (CNIO) is one of the world’s leading cancer research institutions, and many of its findings are directly translated into clinical applications. Their five-year survival rates for common cancers like lung, breast, and colorectal cancer are among the highest in Europe, which speaks to the quality of Spanish healthcare.
A crucial point is that Spain’s public healthcare system covers the vast majority of expensive anti-cancer drugs and treatments. This means that as long as you have a health card, you don’t have to worry about being unable to afford the best treatment. Of course, waiting times can still be an issue, but at least there is hope.
