Hello everyone! For those who’ve recently moved to Spain, I was also pretty confused about the healthcare system here, particularly the legendary ‘free’ medical care. I just successfully applied for my health card, and the process was much smoother than I imagined. I decided to share my experience regarding Spain’s healthcare benefits, hoping to help others who might need it.
What is the Tarjeta Sanitaria?
Simply put, the Tarjeta Sanitaria is Spain’s public health card, acting as your ‘ID’ for accessing the public healthcare system. With it, you can enjoy significant benefits when visiting most public hospitals and local health centers (centro de salud) for consultations and prescriptions, often for free. For instance, regular GP visits, emergency care, hospitalization, and even major surgeries are generally covered by the government as long as they are within the public system. However, please note that services like dentistry and cosmetic procedures are typically not included.

Who can apply for the health card?
The policy used to be more lenient but has become stricter. Generally, you are eligible to apply as long as you are legally entitled to social security. Specifically, this includes the following groups:
- Employees in Spain with a work contract who are contributing to social security.
- Direct family members of an insured person covered by [Spain’s public healthcare].
- Individuals receiving various Spanish benefits or pensions.
- In some special cases, such as pregnant women without resources, the community may also provide temporary medical assistance.
What documents are needed to apply?
The application process isn’t complicated; the key is to have all your documents ready. You’ll need to go to the local health center (centro de salud) assigned to your residential address. It’s best to check their exact address and opening hours online beforehand. Here are the documents I prepared, which you can use as a reference. Note that requirements may vary slightly between different autonomous communities:

After submitting your documents, the staff will review them. Once your application is approved—a key part of the Spanish health insurance system—you’ll typically receive a temporary paper certificate first. The official card will be mailed to your address a few weeks later. Once you have the card, you will be assigned a family doctor (GP), whom you’ll need to make an appointment with for most consultations.
