When I first moved to Spain, figuring out the healthcare system was my biggest headache. I’d heard all the complaints about long waiting times and inefficiency in public healthcare, which made me nervous. However, after living here for several years and experiencing both the public and private systems, I’ve found it’s not so black and white; in some ways, Spanish healthcare is actually excellent. Today, I’m starting this thread to share my personal take on the best parts of the Spanish medical system. This is purely based on my own experience, and I welcome everyone to discuss and add their thoughts!
First off, I have to praise Spain’s family doctor system. It’s truly the cornerstone of the entire public healthcare system. Once you’re registered with social security, you’re assigned a family doctor. For common issues like headaches, colds, fevers, or needing a prescription refill, you just make an appointment with them. In most regions now, you can book appointments through an app, which is very convenient, and you can usually get seen within a day or two. The doctors are very patient and provide an initial diagnosis. For handling minor, everyday health problems, I find the efficiency and convenience of family doctors to be fantastic and completely sufficient.

Specialist Care: Which Departments Excel?
When it comes to specialists, the experiences in the public versus private systems are worlds apart. In my opinion, the standard of healthcare in Spain is top-notch in several specialties, and this is particularly true if you have private insurance, which gives you access to top-tier hospitals in Spain.
Orthopedics and Sports Rehabilitation
Spaniards love sports—football, tennis, hiking, you name it—so their orthopedics and sports rehabilitation medicine is very advanced. I once sprained my ankle playing football and used my private insurance to see a specialist. The whole process, from the X-ray and diagnosis to the subsequent physical therapy, was incredibly fast, and the therapist’s advice was very professional. It really feels like they have a wealth of experience with sports injuries, something you’d expect from the best hospitals in Spain.
Dermatology and Dentistry
These two are arguably the star players of private insurance. For public dermatology and dentistry, the waiting times are a nightmare. But if you have private insurance, the experience is a world of difference. Many insurance plans include one free teeth cleaning per year, and seeing a dermatology specialist is also quick. Several of my friends had their wisdom teeth removed at private dental clinics—quick appointments, great service, and comfortable environments. As for dermatology, private doctors can offer a wider variety of solutions for issues like acne, allergies, or cosmetic consultations.
| Department | Public Healthcare Experience | Private Healthcare Experience |
| Family Doctor | Convenient, wide coverage | Not typically used directly |
| Emergency Room | Long waiting times | Fast, less crowded |
| Orthopedics | Professional, but long waits | Quick appointments, good rehab plans |
| Dentistry | Limited scope, extremely long waits | Comprehensive, great experience |
| Dermatology | Long waiting times | Fast, more options |
My takeaway is this: for minor, everyday illnesses, rely on the public family doctor—it’s easy and saves money. Relying on public specialists for everything is unrealistic. For emergencies or if you want quick access to specialties like orthopedics, dentistry, or dermatology, a good private insurance plan can genuinely enhance your quality of life in Spain.
