When I first came to Spain, I often heard from long-time expats about how great the healthcare benefits were, with medical visits being practically free. After living here for a while, my family and I have experienced both public and private healthcare firsthand, and it’s been quite impressive. But the question of where Spain’s healthcare system actually ranks globally is a fascinating one, so I’ve done some research to discuss it with everyone.
What Do Authoritative Organizations Say?
When it comes to rankings, the World Health Organization (WHO) is probably the most famous. Back in 2000, the WHO published a ranking of the world’s healthcare systems. In that highly influential report, Spain ranked seventh. Although this report on the Spanish healthcare system is a bit dated, it established the general perception of Spain’s healthcare being in the top tier globally. It’s worth noting that the countries ranked ahead of it were established developed nations like France, Italy, and Switzerland.

Besides the WHO, many other organizations have conducted similar studies. For example, Bloomberg releases an annual ‘Health-Care Efficiency Index.’ In the 2020 ranking, Spain was placed third globally, behind only Singapore and Hong Kong. This index primarily measures the ‘bang for your buck,’ meaning Spain’s healthcare system excels in cost management and its contribution to life expectancy. For us taxpayers, this is definitely good news.
Recent Ranking Performance
Rankings are not static; different evaluation criteria yield different results. I’ve compiled a simple table outlining recent Spain healthcare ranking data from various organizations to provide a clearer picture of the current Spain healthcare quality. You can use it as a reference:
| Organization/Report | Year | Spain’s Rank | Evaluation Focus |
| Legatum Prosperity Index | 2023 | 16 | Overall health, including healthcare system, mortality rates, health risks, etc. |
| CEOWORLD Magazine | 2024 | 8 | Medical infrastructure, professional competency, cost, medicine availability, government readiness, etc. |
| Numbeo Health Care Index | 2024 | 1 | Composite score based on website user surveys |
These figures show that while its position fluctuates across different lists, Spain’s overall healthcare quality consistently remains among the world’s best. The fact that Spain can rank first in user-perception-based surveys like Numbeo’s indicates its excellent reputation among the public. That said, rankings are just a reference, and as ordinary residents, we are more concerned with the actual experience of receiving medical care.
Whether looking at the hard data from authoritative institutions or the daily experiences of us residents, Spain’s healthcare level is undoubtedly world-class. It is supported by a robust public system that covers the entire population, complemented by an efficient private healthcare sector. Of course, each system has its pros and cons; for example, long waiting times in public hospitals are a known issue. Have you had any experiences with hospitals in Spain? How was it?
