I’ve just finished moving, and with it comes the task of updating a bunch of addresses. One of the most important is the address on your health card. This is crucial because it determines your assigned family doctor and local health center (Centro de Salud). If you don’t update it, seeing a doctor can become a hassle. I just went through the process, so I’m sharing a breakdown for anyone who needs it.

Why Update Your Health Card Address?
It’s quite simple: Spain’s public healthcare system is geographically based. Your assigned health center depends on your registered home address. After moving, if you don’t update your address, your records will still be linked to your old health center. This means if you need to book an appointment with your family doctor, you’ll have to travel back to your old neighborhood, which is a waste of time and effort. This is especially important for families with children, as vaccinations and regular check-ups are all handled by the local health center. So, updating your address as soon as you move is really important!
What Documents Do You Need?
The required documents are quite straightforward. The main purpose is to prove your new address is valid. Whether you do it online or in person, you’ll need the following core documents:
- Proof of Identity: Your original TIE/NIE and a photocopy.
- Old Health Card: Your Tarjeta Sanitaria.
- Proof of New Address: Your latest Certificado de Empadronamiento (certificate of municipal registration). It’s typically valid for 3 months, so make sure to use a recent one.
- Social Security Proof: Your Vida Laboral (working life report) or any document proving you are registered with Social Security. They might not always ask for it, but it’s better to have it on hand.
My Experience in the Community of Madrid
The rules can vary slightly between Spain’s autonomous communities. I’m based in Madrid, so I’ll be sharing my experience here. Madrid offers both online and in-person options. Since I was in a hurry, I chose to go in person, and it was super efficient.
In-Person Process:
- First, search for your corresponding “Centro de Salud” on Google Maps using your new address.
- Take all the documents I mentioned above and go directly to the reception desk at the health center.
- Tell the staff you want to “Cambiar el domicilio de la tarjeta sanitaria” (change the address on your health card). They will check your ID and your new proof of address.
- Once the information is confirmed, they will update your address in the system on the spot and inform you who your new family doctor is. The whole process took less than 10 minutes—it was very fast! They will give you a temporary paper certificate, and the new physical health card will be mailed to your new home in a few weeks.
Online Process:
If you have a digital certificate or a Cl@ve PIN, you can also apply online, which is more convenient. The process is handled by your local Centro de Salud, and you’ll typically do it through the regional health service’s website. See the table below for a comparison:
