Recently, I’ve seen many friends on the forum asking how to get a health card for their family members, especially for a non-working spouse or children. The key is to apply for a document called the Certificado de Inclusión de Beneficiario, which is what we often refer to as the health card beneficiary certificate. Once you have this certificate, your family members can be registered as your beneficiaries and enjoy the same public healthcare services as you do. I just went through the process for my wife, and I found that it’s not complicated as long as you have all the documents ready. Today, I’m sharing my experience to help anyone who needs it.
Who can be registered as a beneficiary?
First, it’s important to understand that not just anyone can be registered as a beneficiary under your social security. This process is different from obtaining a one-time Spanish Medical Certificate. According to the Social Security Administration (INSS), the main eligible categories are:
- Your spouse or de facto partner.
- Your children or legal dependents under 26 years of age, or over 26 with a disability level of 65% or higher.
- Your siblings.
A key prerequisite is that these family members must be legal residents in Spain and must not be entitled to public healthcare in their own right, for instance, due to their own income or work status. In simple terms, this means they aren’t registered for social security themselves and don’t qualify for a health card through other means. What documents are needed to apply for the beneficiary certificate?
The checklist of documents is, in my opinion, the most crucial part of the process. Having everything prepared will save you a lot of hassle and trips. Here’s what I prepared:

There are two main ways to apply: online or by booking an in-person appointment. I personally recommend applying online, as many government services have been digitized, making it very convenient. You can submit your application through the Social Security’s electronic service platform, Import@ss. After logging in with your digital certificate, Cl@ve PIN, or an SMS code, find the section for “Altas, bajas y modificaciones” (Registrations, cancellations, and modifications), then select “Inclusión de beneficiario” (Inclusion of beneficiary), and follow the prompts to upload your documents. If you don’t have a digital signature, your only option is to book an appointment (Cita) on the INSS website. You’ll need to choose the office nearest to you and bring the original and photocopies of all your documents for the in-person appointment.
The whole process isn’t difficult; the key is to be meticulous.
