Having recently moved to Spain, I’d always heard about the great healthcare benefits here. But I was never sure exactly what that meant, especially when it came to the cost of a comprehensive physical exam. Recently, for various reasons, I’ve had experiences with both the public and private systems, so I can finally shed some light on the topic: Are medical check-ups in Spain actually expensive?
The short answer is: They’re not entirely free, but they’re by no means exorbitant, and your options are very flexible. It all depends on whether you go through the public system or use private insurance.
Public Healthcare System: Free but the “Wait” is Worth It
If you’re registered with social security, most basic tests within the public system are free. For instance, I was having some stomach issues, so I first went to my family doctor (GP), who ordered blood tests and an ultrasound for me. From the doctor’s visit to completing the tests, I didn’t spend a single euro. The downside, as many know, is the waiting. It can take a week or two to get an appointment with your GP, and waiting several months for a specialist or a specific test is common. However, for non-urgent routine check-ups, like those involving medical imaging in Spain, the public system is definitely the most budget-friendly option.

Private Healthcare System: Paying for Time and Efficiency
Later, I wanted a more comprehensive annual check-up and couldn’t wait for the public system’s schedule, so I used my private insurance. The private experience is all about ‘speed’! I found a partner lab and clinic directly through my insurance company’s app and was able to book an appointment within a week. You can choose the tests yourself or have a doctor order them. I have a basic insurance plan with some copago (co-payments), but most of the test costs were covered. If you were to pay entirely out-of-pocket, the price would be higher, but compared to some high-end private check-ups in other countries, I find it quite reasonable.
Reference for Out-of-Pocket/Insured Prices for Common Tests
To give you an idea, I’ve compiled some approximate prices based on my own research and what friends have shared regarding medical check-up costs in Spain. Please note that this is just a general range, and prices can vary between different cities and clinics.
| Test/Procedure | Public System | Approx. Private Out-of-Pocket Price |
| Routine Blood Analysis | Free | €50 - €150 |
| Abdominal Ultrasound | Free | €80 - €200 |
| Chest X-ray | Free | €60 - €120 |
| MRI | Free | €250 - €600 |
| Comprehensive Check-up Package | Not a standard package; tests ordered as needed | €300 - €800+ |
My feeling is that Spain offers a high degree of freedom when it comes to healthcare choices. If you’re not in a hurry and don’t have any clear, urgent symptoms, you can absolutely use the public system for free check-ups. However, if you value efficiency, want a preventative comprehensive exam, or need a quick diagnosis, purchasing a suitable private health insurance plan is a very wise investment. It allows you to access efficient and convenient medical services at a manageable cost when you need them. Have you had any experience with medical check-ups in Spain?
