I finally managed to get my Madrid health card, and it feels like a major life achievement! When I was searching for guides online, I found that much of the information was outdated and everyone’s situation can be a bit different. So, I decided to document my own experience to help others who have just arrived in Madrid and are feeling a bit lost.
Prerequisite: Obtaining Your Social Security Number
Before applying for the health card, the most crucial step is to have a Social Security Number. If you’re a working professional, your employer will handle this for you. If you’re a student with qualifying private insurance, you generally can’t apply for the public health card unless you have an internship contract and your employer has registered you for social security. Therefore, when dealing with healthcare in Spain, the absolute first step is to confirm you have a social security number

Detailed Application Process
1. Booking an Appointment Online
First things first: the Cita (appointment)
! You need to visit the official website for the Madrid community’s health services to book an appointment. Select “Tarjeta Sanitaria,” then fill in your personal details. The system will automatically assign you to the nearest health center (Centro de Salud) based on your registered address (empadronamiento). Pick a convenient time slot and print out the confirmation letter. Although they don’t always ask for it, it’s always safest to have it with you.
2. Preparing Your Documents
This is the most critical part of the process; you can’t miss a single document. When I went to my appointment, the staff checked everything very carefully. Here is the list of documents I prepared, which you can use as a checklist:

Important Reminder: The Certificate of Residence is crucial. Make sure you get a recent one from your local town hall (Ayuntamiento)—don’t use an outdated one!

3. In-Person Application and Card Collection
Arrive a little early for your appointment at your assigned Health Center (Centro de Salud). Find the “Atención al Paciente” (Patient Services) window or a similar desk and submit all your documents. The staff will verify your information and have you fill out an application form. The whole process is very quick, taking about 10 minutes. Once done, they will give you a temporary paper health card on the spot. This document has the same validity as the official plastic card and can be used immediately. The permanent card will be mailed to your home address a few weeks later. I received mine in about three weeks, which was quite fast. Having it makes accessing healthcare in Spain so much easier!
