The premium for a ‘sin copago’ policy is higher, but in theory, all doctor visits, tests, and hospital stays are fully covered by the insurer, so you don’t pay anything out of pocket. ‘Con copago’ policies have cheaper premiums, but you have to pay a small, fixed fee for each medical service, typically ranging from a few euros to over ten. It might not sound like much, but if you need to see a doctor frequently, the Spanish medical costs can add up over a year.

What Services Might Incur Extra Costs?
Even if you have a sin copago policy, don’t celebrate just yet! Some services may still require you to pay out of pocket. The most common example is dental care. Most basic policies only cover cleanings, extractions, and simple check-ups. If you need a filling, root canal, or orthodontics, the insurance company usually only covers a small portion, leaving you to pay the bulk of the cost. Additionally, non-essential cosmetic surgeries, certain psychotherapy sessions, or specific advanced diagnostic technologies might not be covered. Therefore, before undergoing any major treatment, you must first confirm the coverage scope with your insurance provider.
