I just moved to Spain not long ago, and between applying for residency and getting settled, there’s been a ton to do. The biggest headache by far has been getting health insurance. I read all sorts of posts online, but the information was all over the place and just left me confused. I’ve recently done the legwork myself, comparing several companies, and I’ve finally got a clearer picture. I’m starting this post to share what I found, and I welcome everyone to add their own insights or corrections.
How Much Does Health Insurance in Spain Generally Cost?
There’s really no single answer to this question because the prices vary wildly. Many factors affect the cost, mainly your age, the type of plan, the insurance company, and whether dental coverage is included.
To put it simply, for young people like us, a comprehensive sin copago (no co-payment) plan suitable for residency applications generally costs between €45-€65 per month. Of course, the premium increases with age. If you choose a con copago (with co-payment) plan, the monthly fee can be much lower, maybe around €30. This is different from short-term coverage, and you can get an idea of those prices from guides on Spain travel insurance costs. However, if you need to see a doctor frequently, the co-payments can add up, making it less cost-effective. So, be sure to assess your needs carefully before you buy.

A Comparison of Major Insurance Companies
I mainly compared a few of the most common companies on the market: Adeslas, Sanitas, Asisa, and DKV. They are all major Spanish companies with extensive service and medical networks. Here’s a general overview based on my personal inquiries and research:
| Insurance Company | No Co-payment Monthly Fee | Features & Personal Impressions |
| Adeslas | €50-€60 | Has a very large network of clinics and hospitals. It’s a household name, and many of my friends use it. |
| Sanitas | €55-€65 | Known for its high-quality service and great reputation, so its price is slightly higher. |
| Asisa | €45-€55 | The price is quite competitive, offering good value for money. It’s also a well-established company. |
| DKV | €48-€58 | Originally a German company, so it feels very meticulous with its procedures and reimbursements. Quite a few people use it too. |
Here’s a piece of advice: don’t just focus on the price! Saving a few euros might mean a smaller medical network or the exclusion of certain special tests. Before you buy, be sure to read the policy terms carefully, especially the part about the ‘waiting period’ (periodo de carencia). Many insurance plans have a waiting period of several months for non-emergency major surgeries or examinations. I hope this information helps anyone who’s struggling with this decision. Let’s all stay healthy and enjoy life in Spain!
