Hi everyone, I just moved to Spain and have been swamped with everything from getting my residency permit to renting an apartment. The biggest headache for me has been health insurance. The information online is all over the place, and different friends have told me different things. After spending a lot of time researching, I’m finally getting the hang of it. I’m starting this thread on Spanish health insurance to share what I’ve learned, and I welcome any corrections or additions from the veterans here!
Public Healthcare vs. Private Insurance
First, you need to understand that Spain has two healthcare systems: public and private. If you have a work contract, your employer is required to register you for social security, which gives you access to public healthcare. You take your social security card to your local health center (centro de salud) to register, and they’ll assign you a family doctor (GP). However! The main feature of the public system is the wait. It’s common to wait for months for an appointment with a specialist or for a diagnostic test. While it’s free, the time cost is significant. This is why many people, even those with social security, choose to get private insurance as well.

Why Do Many People Recommend Private Insurance?
For many of us newcomers, such as those applying for a non-lucrative visa, a job-seeking residency, or as students, getting private health insurance is a mandatory requirement for the residency application. Aside from immigration needs, the biggest advantage of private insurance is speed! It’s efficient! If you’re feeling unwell, you can book an appointment with a specialist directly through the insurance company’s app, usually within a few days. You also have a wider choice of hospitals, which generally offer better environments and service. Many also provide English-speaking staff, which is a huge plus for those of us who aren’t fluent in Spanish yet. Of course, the downside is the monthly premium, but you get what you pay for.
How to Choose Among the Major Insurance Companies?
There are many insurance companies in Spain, but the major ones are Adeslas, Sanitas, DKV, and Asisa. From my research, they are quite similar, with the main differences being their network of affiliated hospitals, premium costs, and some special services. For example, some companies have strong dental coverage, while others include services like psychotherapy or fertility treatments. The choice really depends on your personal needs and budget. I recommend comparing quotes and policy details from several companies. I’ve made a simple comparison table below to give you a quick overview:
| Insurance Company | Key Features | Best For |
| Adeslas | Widest hospital network, a major national brand | Those who prioritize broad coverage and want access to affiliated hospitals everywhere. |
| Sanitas | Good service and app experience, includes many high-end private hospitals | Those who value service quality and patient experience, with a slightly higher budget. |
| DKV | German-based, known for meticulous service and competitive pricing | Those looking for good value for money and are interested in health management services. |
| Asisa | Relatively affordable prices, comprehensive basic coverage | Those on a limited budget, primarily needing insurance for residency requirements or basic medical care. |
Finally, I want to say that buying insurance is really about buying peace of mind. Don’t try to save a little money by getting a non-compliant or incomplete plan. It would be a huge problem to find out something isn’t covered right when you need it. For those applying for residency, it’s crucial to buy a policy that is sin copago—that is, a comprehensive plan with no co-payments (you don’t pay extra each time you see a doctor) and no waiting periods. I hope this information is helpful to everyone, and I’d love to hear your thoughts and experiences too!
