It often feels like you have two choices: enduring the endless waiting times of the public system or paying the hefty price for commercial private insurance. I thought so too, until a local friend told me about Spain’s third option: non-profit health insurance companies.
At first, hearing ‘non-profit’ made me think of a charity. But I soon learned they are quite different from the commercial insurance companies we all know. In simple terms, these companies are mutually owned and managed by doctors and their insured members. Their goal isn’t to maximize profits for shareholders, but to use the premiums collected to provide the best possible medical services for all members. That’s why their prices are often much more affordable than purely commercial private insurers like Adeslas or Sanitas, without compromising on service quality or coverage!

Common Non-Profit Health Insurers
You might not be familiar with these names, but they are quite popular among locals in Spain. Some of the most well-known ones include:
- Igualatorio Cantabria: This is arguably the pioneer of the non-profit model, with a very long history.
- Asisa: Many people might think Asisa is a typical commercial insurer, but its foundation is also based on a non-profit, cooperative model.
- DKV: DKV Seguros is another company that operates on a non-profit basis in Spain, with a special focus on promoting preventive medicine and healthy lifestyles.
The biggest difference between them and purely commercial insurers is that if these Spanish non-profit healthcare companies have a surplus at the end of the year, the money isn’t distributed to shareholders. Instead, it’s reinvested into improving medical equipment, expanding their network of partner hospitals, or even lowering premiums for the following year. Sounds much more reliable, doesn’t it?

If you have a stable job and are registered with social security, the public system is sufficient as a basic safety net.
