Shortly after arriving in Spain and getting my residency sorted, the top priority was to apply for a health card. After all, when you’re abroad, having healthcare coverage for even a simple headache or fever provides great peace of mind. I searched online and found a lot of conflicting information, so I ended up visiting the Social Security office and the local health center myself to figure it all out. Today, I’m sharing my up-to-date experience to help anyone struggling with this process.

Preparing Your Core Documents
The core requirement for the health card application is to prove that you are legally residing in Spain and are entitled to public healthcare. This entitlement typically comes from you or a family member contributing to the Social Security system. Therefore, the process generally involves visiting the Social Security office first, followed by your local health center.
Step 1: Obtain the Document from the Social Security Office
You need to go to the Social Security Administration (Tesorería General de la Seguridad Social - TGSS) to get a document certifying your right to public healthcare. This document is called the ‘documento acreditativo del derecho a la asistencia sanitaria’. Before you go, make sure to bring your residence card (TIE), passport, and your most recent proof of address (certificado de empadronamiento). If you are working, it’s also a good idea to bring your work contract or your ‘vida laboral’ (work life report). The staff will verify your information and print the document for you on the spot. Once you have this paper, you’re halfway there!
Step 2: Go to the Health Center
With the document from the Social Security office, you can now go to the community health center (Centro de Salud) corresponding to the address on your proof of residence (empadronamiento). Remember, you must go to the one assigned to your area; they won’t process your application if you go to the wrong one. If you’re unsure which one it is, you can search on Google Maps for ‘Centro de Salud’ followed by your postal code. Once at the health center, tell the receptionist you want to apply for the ‘Tarjeta Sanitaria,’ and they will guide you.

After submitting all the documents, the staff will give you a temporary paper certificate, which allows you to see a doctor right away. The official plastic health card will be mailed to your home address within a few weeks to a month. The whole process isn’t complicated; it just requires visiting two different places.
