Just moved to Spain? The last thing you want is to get sick. I’ve always been pretty healthy, but with the recent change of seasons, I felt a bit under the weather. I looked into the hospital appointment process in Spain and found it’s quite different from what I’m used to, with its own set of rules. Here’s the information I’ve gathered, and I hope it can help anyone who needs it.
Public Hospitals
Public healthcare is the go-to option for most legal residents, provided you’re registered with social security and have a public health card (tarjeta sanitaria)
. Typically, you don’t go directly to a major hospital; you start at your assigned local health center (Centro de Salud).
Booking an Appointment with Your Family Doctor
The first step is always to book an appointment with your family doctor (GP or médico de cabecera). Your GP will provide an initial diagnosis and handle minor issues on the spot. If you need further tests, they will refer you to a specialist or a larger hospital, which requires a Cita Previa. Here are the main ways to book:
- By Phone: Call the number on your health card or the one provided by your Centro de Salud. Follow the automated prompts and enter your health card number and other details.
- Via App or Website: This is my top recommendation—it’s super convenient. Each autonomous community has its own health service app, such as ‘Cita Sanitaria Madrid’ in Madrid or ‘Salud Responde’ in Andalusia. Just download the app, link it to your health card, and you can choose your preferred time and doctor.
- In Person: Go directly to the reception desk at your local health center and ask the staff to book an appointment for you.

Private Hospitals
If you have private health insurance, seeing a doctor is much more straightforward. Private hospitals are generally more efficient, with shorter waiting times. The booking process is also more direct.
Directly Booking a Specialist
With private insurance, you don’t need a referral from a family doctor. You can directly choose and book an appointment with a specialist from your insurance provider’s network of hospitals and doctors. Here’s how:
- Insurance Company’s App/Website: Log in to your insurance provider’s account (e.g., Sanitas, Adeslas, DKV). You can search for doctors and either book online directly or find their contact information.
- Contacting the Hospital or Clinic Directly: Find the phone number, WhatsApp, or website of the private hospital or clinic you want to visit and contact them to book a Cita. Remember to mention your insurance company and policy number beforehand.
While the public system is free, its process is more structured and requires going through a family doctor first.
