Hi everyone, I’ve been in Spain for just over a year and recently decided to get a gynaecological check-up, specifically for an HPV test. Back in my home country, it was a standard part of my annual health check, but I haven’t had one since moving here, and I’m feeling a bit anxious about it.
I’ve been digging through the forums, but the information seems a bit scattered. So, I decided to start this thread to ask for your experiences and to consolidate what I’ve found as a reference for anyone with the same questions. The main thing I want to know is: how much does an HPV test actually cost in a Spanish hospital? Is there a big difference between public and private options?

Public Hospitals vs. Private Clinics
First, let’s talk about the price, which is everyone’s main concern. Based on conversations with friends and my own online research, here’s the general situation:
Public System
If you have public health insurance (social security), an HPV test is completely free. The catch, however, is that you can’t just get one on demand. You typically need to book an appointment with your GP (General Practitioner), who will then provide a referral to a gynaecologist if they deem it necessary. After that, you’re in for a long wait. The waiting period could be a month or as long as three to five months, depending on the resources in your autonomous community. Furthermore, the public system adheres to strict screening guidelines. For example, tests are generally recommended only for women over 25 and are repeated every 3 or 5 years, not annually. This contrasts with services like getting the HPV vaccine in Spain, which has its own procedures. This means getting a simple annual check-up just for peace of mind is quite difficult through the public system.
Private Clinics
If you don’t want to wait or don’t have public health insurance, a private clinic is your best bet. The advantages are that it’s fast and convenient. You can book an appointment with a gynaecologist directly online or by phone and usually be seen within a week. The downside, of course, is the cost. I inquired at a few private clinics in Madrid and Barcelona, and the prices varied quite a bit. Here’s a rough price range I’ve put together for reference:
| Service | Estimated Price Range | Notes |
| Gynaecological consultation only | 80€ - 150€ | Just a conversation with the doctor, no specific tests. |
| Consultation + HPV/Pap smear | 150€ - 250€ | This is the most common package. |
| Full gynaecological check-up | 250€ - 400€+ | Includes HPV test, ultrasound, and other exams. |
If you have private health insurance, it’s a different story. Most private insurance plans cover an annual gynaecological exam, and you might only have to pay a small copago (co-payment) or nothing at all, depending on your policy. So, if you have private insurance, pull out your contract and check the details!
If money isn’t an issue, you value convenience, or you have private insurance, go private. If you want to save money, aren’t in a hurry, and meet the public screening criteria, use the public system. Personally, I didn’t have the patience to wait for the public system, so I ended up booking a private appointment. It cost me €180, and while it was a bit pricey, I got my results quickly. It was worth it for the peace of mind. I hope my experience helps, and I’d love for others to share their information too!
