I recently moved from Andalusia to Madrid, and my top priority was to sort out my medical card. I searched the forums and found many discussions about applying for and using a medical card in a different region, but the information was a bit scattered. Since I just went through the process, I’m writing down my experience to help others who might need it.
Can I still use my old medical card after moving?
This is probably the biggest question on everyone’s mind. In theory, your healthcare rights are valid nationwide, so using your old card for emergency services at any public hospital is not a problem for making a medical appointment. However, for routine healthcare, like booking an appointment with your family doctor (médico de cabecera), you must do it at the health center (Centro de Salud) where you are registered. Once you move to a new autonomous community, you need to get a new medical card so the system can assign you to a health center near your new address.
So the conclusion is: It’s fine for emergencies, but you need a new card for routine doctor visits
! Don’t be lazy and think you can keep using the old card indefinitely; you’ll be in a real bind when you actually need to book an appointment. This is for the public system, of course, not for things like premium medical cards

How do you apply for a new medical card?
The process is actually not complicated; the key is to have all your documents ready. The most crucial part is to first get your certificate of residence (empadronamiento) in your new city. Once you have the new empadronamiento, you can apply for the medical card. Here are the steps:
- Find your assigned health center: Based on your new address, search for “Centro de Salud” on Google Maps to find the one closest to you.
- Prepare your documents: This is the most critical step. You can refer to the table below. Note that requirements may vary slightly by autonomous community, as can the process for replacing a medical card, but generally, these are the documents you’ll need.

The official plastic card will be mailed to your new address in a few weeks.

The whole process was smoother than I expected. The staff in Madrid were quite efficient, and I didn’t have to wait long. However, I still recommend going early to avoid peak hours. Also, make sure you get your empadronamiento first
