After settling in Spain, one of the first and most important things to do is get your health card. I found the guides online to be a bit fragmented, but I finally managed to get mine after going through the process myself. I’m sharing my fresh experience with new friends on the forum, hoping to help you avoid some common pitfalls.
What is a Health Card?
Simply put, the health card, officially called the Tarjeta Sanitaria Individual (TSI), is your ID for the Spanish public healthcare system. With it, visits to your local health center and prescriptions are mostly free or heavily discounted, making it an absolute essential for life here. The design of the card may vary slightly by autonomous community, but the function is the same, as is the process for booking a medical appointment.

Key Steps to Apply for a Health Card
The whole process isn’t too complicated, but you must have all your documents ready. Making a trip for nothing because of missing paperwork is really frustrating.
1. Get a Social Security Number
This is a prerequisite; you can’t proceed without this number. If you are employed, your company will apply for it on your behalf. If you are a student or here on a family reunification visa, you’ll need to apply for it yourself at the Social Security office (Tesorería General de la Seguridad Social). Applying online is also very convenient now and only takes a few minutes.
2. Obtain Your Proof of Residence (Empadronamiento)
This certificate is extremely important. You’ll need it not just for the health card application but for many other official procedures as well. You can get it at the town hall (Ayuntamiento) for your residential area. Remember to book an appointment in advance. The certificate is typically valid for 3 months, so make sure yours isn’t expired.
3. Prepare Your Documents and Go to the Local Health Center
Take all your documents to the health center (Centro de Salud) assigned to the address on your empadronamiento certificate. You can’t just go to any health center; it has to be your designated one. If you’re unsure which one it is, you can search on Google Maps for “Centro de Salud” followed by your postal code. The process is quick. The staff will check your documents, enter your information into the system, and usually give you a temporary paper certificate on the spot. The official plastic card will be mailed to your home later.

Once it’s done, you’ll have peace of mind! For any minor ailments like a headache or fever, you can go directly to your local health center to see your assigned family doctor, which is very convenient.
