The process is actually very simple. Just take your health card (tarjeta sanitaria) and ID to the front desk of your assigned health center (Centro de Salud). You can tell the staff, “Quiero hacer la firma del equipo médico” (I want to do the medical team signature) or “Quiero elegir un médico de familia” (I want to choose a family doctor).
