When you first arrive in Spain, besides finding a place to live and getting a phone plan, the most important thing is to get your health insurance sorted. While public healthcare is free, the waiting times for appointments can be incredibly long. For students and working professionals, time is money! That’s why getting private health insurance is pretty much standard practice. But with so many companies on the market—Sanitas, Adeslas, Asisa, DKV… it can be overwhelming. I’ve recently spent a lot of time researching them, and today I’m sharing my findings to help anyone who’s struggling to decide.
A Brief Analysis of the Pros and Cons of Major Providers
I’ve focused on comparing the four most common providers on the market: Sanitas, Adeslas, Asisa, and DKV. These companies basically dominate the private healthcare market in Spain and have good service networks and reputations.

Sanitas
Pros: Sanitas is perhaps the most famous name among expats when it comes to Spanish health insurance, and the service experience is genuinely great. Their app is exceptionally well-made, allowing you to book appointments, see a doctor, and check results all in one place, which is very convenient. Their online video consultations are very responsive, making them a lifesaver for minor ailments or getting prescription refills. Many clinics have English-speaking staff, which is very friendly for those with a language barrier.
Cons: Expensive! Without a doubt, Sanitas’s premiums are typically the highest among these providers. Also, note that some basic plans, which you might find while looking into topics like Spanish medical recruitment, may not include dental coverage, requiring an additional purchase. This is an important point to verify.
Adeslas
Pros: Adeslas is the company with the largest market share in Spain. Its network of partner clinics and doctors is the most extensive. Whether you’re in a big city or a small village, you can almost always find one of their affiliated medical centers. It offers great value for money; for similar coverage, its premiums are usually cheaper than Sanitas.
Cons: Due to its large user base, it can be difficult to get an appointment with renowned specialists in popular cities. Personally, I find their app and online service experience not as smooth as Sanitas’s; it can be a bit glitchy at times.
Asisa & DKV
Pros: Asisa and DKV are considered second-tier providers, but they are still very strong contenders. They often feature highly competitive pricing plans, especially for insurance required for residency renewal, offering excellent value. DKV has German origins, and its service quality and claims process are known for being rigorous and well-structured.
Cons: Compared to the first two, they have fewer of their own clinics and rely more heavily on a network of partner clinics. In smaller towns, your choice of Spanish health insurance providers and doctors might be limited. Additionally, their brand recognition may not be as strong, but the service itself is reliable.
Core Services Comparison
To make things clearer, I’ve created a simple table to compare some of the most important aspects:
| Feature | Sanitas | Adeslas | Asisa | DKV |
| Price Level | High | Medium | Medium-Low | Medium-Low |
| Medical Network | Extensive | Most Extensive | Wide | Wide |
| App Experience | Excellent | Good | Average | Good |
| Chinese Service | Often Available | Limited | Rare | Rare |
| Dental Coverage | Mostly Add-on | Included in some plans | Included in some plans | Mostly Add-on |
For those not concerned about the budget and seeking top-tier service and convenience, Sanitas is the way to go. If you’re looking for the best value for money and a wide medical network, Adeslas is the top choice. If you’re on a limited budget or primarily need insurance for residency renewal, the basic plans from Asisa and DKV are excellent options. Before buying any insurance, be sure to read the terms and conditions carefully, especially the rules regarding waiting periods and co-payments, to avoid any pitfalls later on. I hope my analysis helps, and I welcome other users to share their experiences!
