Why do you need a health card?
This card is incredibly important! Simply put, it’s your proof of identity within the Spanish public healthcare system. With it, you can book appointments with a family doctor, visit public hospitals, and get most prescription medications at a subsidized price. Without it, a doctor’s visit could result in a massive bill. Whether you’re working or studying, as long as you have a legal residence permit and are registered for social security, getting your Madrid health card should be your top priority.

Required Documents
When I went to apply, the staff mainly checked for the following items. I recommend bringing both the original documents and photocopies, just to be safe:
- Proof of Address (Certificado de Empadronamiento): This is crucial as it proves which community health center you belong to. Note its validity period; it usually needs to be issued within the last 3 months.
- Residence Card (TIE): This is your official ID and must be valid.
- Social Security Number Document: A document that proves you are registered with the social security system. It’s a paper that shows your social security number.
Some people might also be asked for their work contract, but I wasn’t. I’d suggest bringing all related documents you can think of, like your passport, school enrollment letter, etc., just in case.
Application Process and Location
The whole process isn’t actually complicated; the key is to go to the right place with the right documents.
- Find Your Health Center: First, you need to find your designated community health center (Centro de Salud) based on the address on your empadronamiento. You can simply search on Google Maps for “Centro de Salud” followed by your postal code or street name.
- Make an Appointment: Some health centers require an online or phone appointment, while others allow you to just walk in and queue at the front desk. I was a bit lazy and just went on a weekday morning; it wasn’t crowded, and I was attended to after a short wait. I recommend checking with the center beforehand to avoid a wasted trip.
- Submit Your Documents: When it’s your turn, give all your prepared documents to the staff. They will enter your information into the system and ask you to verify it. If everything is correct, they will give you a temporary paper version of the health card on the spot, which includes your personal details and your assigned family doctor.
You can use this temporary paper certificate to book doctor’s appointments right away. The official plastic card will be mailed to your home address in a few weeks.
