For students who have just arrived in Spain, are you worried about how to see a doctor? Don’t worry, today I’m sharing my full experience as a student applying for the public health card in Madrid. It’s a tried-and-tested process that I hope will help everyone!
Many people might think that the private insurance bought by students is enough, but having a public health card is a great backup. After all, the public healthcare system has broad coverage and is often free. Most importantly, having it means you are recognized by the Spanish social security system, which gives you more peace of mind! Plus, the application process for free healthcare in Spain is simpler than you might think. As long as you have all the necessary documents, you can usually get it without any issues.
Application Process
Step 1: Obtain Your Social Security Number
This is a prerequisite for applying for the health card. You’ll need to go to the Social Security office first. Nowadays, many regions allow you to apply online, which is very convenient. You will need to fill out the Modelo TA.1 form and attach scanned copies of your residence card (TIE), passport, and Empadronamiento certificate (proof of address). After submission, you should receive your social security number by email within a few days.
Step 2: Book an Appointment (Cita) at Your Health Center
Once you have your social security number, you need to go to your assigned local health center (Centro de Salud) to submit your documents. First, you must book an administrative appointment (Cita) online. Search Google for “Cita Previa Sanidad + your city,” go to the official website, select “Solicitud de Tarjeta Sanitaria,” and follow the instructions. You will typically need to enter your personal information.

Step 3: Prepare and Submit Your Documents
On the day of your appointment, bring all your materials to the Centro de Salud. The staff will check your documents. If everything is in order, they will give you a temporary health card certificate on the spot, which includes your information and your assigned family doctor. You can use this paper to see a doctor. The official physical card will be mailed to your home address in a few weeks or a couple of months.
| Required Documents | Notes |
| Original and photocopy of your passport | All pages, especially the visa page |
| Original and photocopy of your residence card (TIE) | Front and back |
| Empadronamiento certificate | Must be valid, usually issued within the last 3 months |
| Social security number assignment document | The email or document you received with your social security number |
| Appointment confirmation | Printed or saved on your phone |
The whole process isn’t that complicated; the key is to be patient and meticulous. The specific requirements might have slight variations depending on the autonomous community, so it’s a good idea to check the official health system website for your region before you start. I wish you all success in getting your health card and a healthy and happy time studying and living in Spain! ¡Mucha suerte
