Hello everyone! I’ve been in Spain for a while now, and after lurking on forums reading all the great guides, today I want to share something everyone is surely concerned about—seeing a doctor in Spain! When I first arrived, I often heard from senior students that Spain has ‘free healthcare,’ but I was always a bit unsure about how it actually works without experiencing it myself. Recently, due to some seasonal allergies, I went through the entire process firsthand. So today, I’m here to break it all down for you and also provide a complete guide to applying for the health card.

So, is the legendary ‘free healthcare’ real? The answer is: for most international students, yes, it is
! But this doesn’t mean you can just walk into a hospital with your passport. This ‘free’ access is based on having Spanish public health insurance. As long as you have successfully applied for a Social Security Number and a health card, seeing a doctor and getting prescriptions within the public healthcare system is basically free of charge. For students like us with limited income, this is a huge blessing!
The Two Core Steps to Getting Your Health Card
So, how do you apply for this amazing health card? It’s not complicated and mainly involves two steps: applying for a Social Security Number and applying for the health card. I’ll list the specific steps and required documents for you below.
Step 1: Apply for a Social Security Number (Número de Seguridad Social)
This is the first and most crucial step of the whole process. You need to go to your nearest Social Security office (Tesorería General de la Seguridad Social - TGSS) to apply. I highly recommend booking an appointment (Cita Previa) online first, otherwise you might make a wasted trip—a key point in any guide for the Spanish health card. Especially since the pandemic, many offices now require an appointment.
| Required Documents | Notes |
| Passport (original and copy) | Copy of all pages, including the visa page |
| TIE/Residence Card (original and copy) | Copy of both front and back |
| University acceptance letter or proof of enrollment | To prove your student status |
| Empadronamiento (Proof of address) | Usually required to be issued within the last 3 months |
| TA.1 Application Form | Can be downloaded from the Social Security website and filled out in advance |
Once you submit the documents and the staff verifies them, you will usually be given a paper on the spot with your Social Security Number on it. Be sure to keep this document safe!
Step 2: Apply for the Health Card (Tarjeta Sanitaria)
After getting your Social Security Number, you can go to your local health center (Centro de Salud) to apply for the health card. Again, bring your Social Security Number document, passport, TIE, and Empadronamiento. Go to the reception desk at the health center and tell the staff you want to apply for a Tarjeta Sanitaria, and they will guide you. After completing the process, they will first give you a temporary paper certificate, which you can use to see a doctor right away. The official plastic card will be mailed to your home address a few weeks later. Receiving the card feels like getting a ‘health safety net’ in Spain, which brings great peace of mind! I hope my sharing helps those in need. Wishing everyone in Spain good health—the best scenario is never having to use this card!
