Finding out you’re pregnant is a whirlwind of excitement and a little bit of anxiety! Based in Madrid, my husband and I immediately started looking into everything related to prenatal care and giving birth here. One of our biggest concerns was the cost. After doing some research and talking to experienced friends, I’m sharing what I’ve learned about the costs of prenatal care in Spanish hospitals. I hope this helps other expectant mothers out there.
In Spain, the healthcare system is divided into public and private sectors. The cost of your prenatal care will largely depend on which system you use.
Public Hospital Prenatal Care: Almost Free
If you or your partner are employed and contributing to social security (seguridad social), then congratulations! The public system covers all routine prenatal check-ups, lab tests, and the final cost of childbirth. This means it’s essentially free. You’ll need to visit your local health center (centro de salud) to see your family doctor (médico de cabecera). Once your pregnancy is confirmed, they will refer you to a midwife (matrona) and an obstetrician/gynecologist (ginecólogo), who will then schedule all your appointments according to the standard protocol.
The advantage of the public system is that it saves money and guarantees a high standard of medical care. The downsides can include longer waiting times, fewer ultrasound scans, and a service experience that might not feel as “customer-first.”

Private Hospital Prenatal Care: Costs Depend on Your Insurance
Many people opt for private health insurance for a better service experience, shorter waiting times, and more testing options. If you have private insurance that covers prenatal care and childbirth, most of the costs will be covered by the insurance company. However, it’s crucial to note that many private insurance policies have a waiting period known as “carencia”, which is typically 8 to 10 months. This means you must have purchased the insurance before becoming pregnant; otherwise, the insurance company may not cover the expenses. A friend of mine learned this the hard way when she tried to buy insurance after getting pregnant.
If you choose to pay out-of-pocket at a private hospital, the costs can be quite substantial. Fees vary between hospitals and doctors, but I’ve consulted a few and compiled a list of reference prices for common procedures to give you a general idea:

Going through the entire process of prenatal care in Spain on a completely out-of-pocket basis at a private facility can easily cost several thousand euros.
